Once completed, you can then send the form to Mr Roger King (details attached on the form) with your annual fees. The form is easy to understand and will explain how the membership works.
New and existing members up until December 2009 stand a chance to win a Chelsea 2009/10 Home or Away strip. This draw will take place in early January 2010.
